We will require a copy of your child's immunization records prior to the first day of the session. Refund Policy *

When you register, you are committing for the full school year. If your plans change and you want to cancel your registration before May 1st, we will issue a full refund. If you want to cancel your registration after May 1st, we will do everything that we can to help you to find a family to take your child’s spot in class for the school year. We will issue a refund if a family does take your spot in the class. If your child attends the Fall Session but you need to withdraw him/her from the Winter and Spring sessions due to unforeseen circumstances, we will do everything that we can to fill the spot for you and will issue a refund if we are able to find another family to take your child’s spot in class. We do not enroll new students for the Spring Session only so if your child has to withdraw from the program for just the Spring Session, we would not fill your child’s spot and you would not receive a refund for that session. We normally have a waitlist for our classes and a number of families often add extra mornings to their child's enrollment for the Winter or Spring Sessions so I am optimistic that we would be able to find a family to take your child’s spot in class if your plans change after May 1st for the full school year or before the Winter Session begins in January 2023. However, a refund is not guaranteed after May 1st.

Please read the information below and then sign the agreement by typing your name in the box below: LIABILITY AND ASSUMPTION OF RISK I understand that my child will be attending Creative Groove Studio classes at Mile Square Theatre (1400 Clinton Street, Hoboken) at his, her or my own risk and I will not hold the officers, directors, independent contractors, volunteers, or employees of The Creative Groove Studio, LLC or Mile Square Theatre responsible for any injuries or damages received by the aforementioned student. I acknowledge my acceptance of this Assumption of Risk by agreeing to and allowing my child to participate in such classes. MEDICAL EMERGENCIES I hereby authorize the employees of Creative Groove Studio, an identified additional parent/guardian on the registration form, or the identified Emergency Contact to act as my agent and to consent to medical, surgical, or dental examination and/or treatment that may be necessary. I hereby authorize Creative Groove Studio's employees to perform any acts which may be necessary or property to provide emergency health care of my child in the event that the parent/guardian and/or Emergency Contact cannot be reached, including consent to and authorization of medical procedures by qualified, licensed physicians, dentists, hospital or emergency medical personnel, as they, in the exercise of their profession and in their sole discretion, may deem necessary. I understand that I am responsible for all costs and expenses of such medical treatment.

What will happen if classes have to be canceled due to COVID issues?* *

If we have to cancel classes due to local COVID issues, we will attempt to schedule make-up classes later during the school year. However, if that is not possible, we will issue a 50% refund for canceled classes. We are unable to offer 100% refund as we will continue to pay teachers and rent during the period when classes are canceled.
